Eyelid Rejuvenation Surgery ( Blepharoplasty)
Upper Eyelid Surgery
In the 3rd -4th decade excess skin develops in the upper eyelid (dermatochalasia). The excess skin folds on itself to distort the youthful upper eyelid (eyelid hooding). This reduces the attractiveness of the eyelid and causes difficulties with makeup application. In severe cases the excess skin may hang over the eyelid margin, pressing on the eyelashes.
Eyelid surgery is a technique to carefully remove the excess skin and improve the contours of the upper eyelid. This can be performed under local or general anesthesia. The procedure takes
approximately one hour as a day-case procedure and there is very little down time. Many patients report mild or no discomfort in the days following surgery.
Eyelid surgery involves precise incisions on the upper eyelid to remove excess skin. The goal is to place the incisions in a manner that on healing the scar will be hidden in the eyelid fold. This is the
part of your eyelid when you look into the mirror that is hidden in a recess.
At your consultant Mr Blythe will carefully examine and assess your eyelids. A proportion of heavy looking eyelids is due to descent of your brow ( brow ptosis). With age the forehead lengthens and the brows fall from their youthful position to a lower position. If you consider the eyelid skin as a hammock, if you imagine one of the palm trees has moved closer to the second palm tree the
hammock drops and looses its ideal position. With the brow dropping the eyelid skin appears excessive. In these cases a brow lift is indicated to gently lift ht brows and lift the upper eyelid skin
(moving the palm tree further away)

Lower Eyelid surgery
Often 5-10 years after the upper eyelids start showing signs of skin excess the lower eyelids display ageing features. These features include skin excess with gathering of skin beneath the eye,
and wrinkles. Cushion the eyeball (globe) below the eye is a mattress of fat and above the eyeball a duvet of fat. The fat is secured in position by fine fibrous bands and the eyelids. With ageing the
fat is pushed forwards into the eyelids. This becomes visible as bags.
There are many approaches to managing the lower eyelid depending on the clinical features. For patients where fat herniation is the predominant feature, a gentle incision can be made behind the
eyelid (transconjunctival incision) and the fat carefully removed.
In cases with excess skin, an incision image just below the lower eyelashes and the excess skin removed. The eyelid is tightened fat removed as necessary and the wound precisely closed. In
mild cases, a simple “pinch” blepharoplasty is performed. Both approaches can be performed under local anaesthetic, sedation or general anaesthesia.
Surgery takes approximately one hour for a pair of eyelids. Downtime is minimal.
